If you are applying to take your child out of school during term time, you will require the following form:
REQUEST FOR CHILD ABSENCE DURING TERM TIME
Please note: Taking your child out of school during term time could be detrimental to your child’s
educational progress. The Headteacher will NOT authorise time off for holidays (in accordance with the Pupil Registration Regulations 2006).
If you require the school to administer medication to your child at any point during the school day, the following form must be completed and delivered to reception with the medication:
